The Safety Net Association of Primary Care-Affiliated Providers of WNY, Inc. (SNAPCAP) serves as an advocate for its members in Western New York, addresses concerns at the policy level, and offers leaders a forum for sharing information and managing change. 



Through the collaboration of its members, SNAPCAP works to achieve:

  • Appropriate stakeholders engaged in the safety net planning process

  • Established communication structures that ensure effective coordination and mutual support

  • Identification, collection, and dissemination of information about projects that may impact both providers and the community

  • Recommendation of goals, objectives, and timelines for achieving improved safety net services and access

  • Coordinated and supportive pursuit of funding and other resources

  • Advocacy for public policy that is supportive of increased access to primary care and improved primary care safety net services

  • Improved public awareness of primary care safety net services in the community 

  • Strong, sustainable development of patient-centered medical homes



Cooperation and coordination among safety net providers can improve services and strengthen individual organizations. State and Federal governments place many demands on these providers. Each member organization takes advantage of the shared lessons other organizations have learned while seeking to achieve these standards.
